The Accommodation Outreach Program
The Accommodation Outreach Program (AcOP) supports clients who live independently or with family or friends in Maitland, Cessnock, Merriwa, Dungog, Muswellbrook, Singleton, Scone, Port Stephens or Upper Hunter Local Government Areas.
The Program offers a range of flexible and responsive services based on the individual needs of clients and their families and carers.
Aims of the Accommodation Outreach Program
The Accommodation Outreach Program aims to assist clients to maintain and enhance their independence through informal and responsive support options identified through individual planning and review. Priority is given to people with a disability living with ageing carers.
The Accommodation Outreach Program assists Clients:
- To maximise independence and participation in the community.
- To enhance the quality of health and wellbeing.
- To achieve more independent living arrangements.
- To negotiate problems that may arise.
AcOP Activities & Services
Independent Living Training
- Enabling access and use of community resources such as shops, banks and public transport
- Training with mobility, orientation and use of adaptive equipment
- Skills development to manage day to day tasks such as cooking and cleaning
Personal Assistance
- Support with personal care, hygiene and medication
- Facilitating access to medical services
Social and Recreational Activities
- Enabling access and maintenance of community contacts and social relationships
- Support and training in the management of emotions and personal responses
Family and Carer Support
- Assistance to plan for the future needs of the client
- Consideration of the needs of the family and carers when planning services and activities
